
If you or a loved one are admitted to hospital, help and support should be organised before you are discharged. The support may involve:
- Arranging extra help - paid home help or visits from the district nurse
- Any equipment required is fitted
- Home adaptations are made
Care Plan
The staff member in charge of your discharge will assess you to create a care plan. You will have input into that plan to make sure you have everything in place when you leave. The care plan will include:
- Who is responsible for your care and how to contact them
- When and how frequently will you care
- The care and treatment you need when you get home
