When it comes to providing care and support for individuals facing health or social challenges, care assessments play a crucial role in determining the specific needs and requirements of you or your loved one. This comprehensive evaluation is designed to create a tailored care plan that addresses unique circumstances. An assessment is a chance to talk about any issue you are facing and get the personalised assistance you need.

What is a Care Assessment?

A care assessment, also known as a needs assessment or care needs assessment, is a systematic and thorough evaluation that identifies an individual's physical, emotional, and social needs. The goal is to determine the level of support required to enhance the person's overall well-being and quality of life. Care assessments are often conducted by healthcare professionals, social workers, or care coordinators, depending on the specific context and the nature of the individual's needs. You can expect your chosen Care provider to complete this as part of their initial assessment, once your care connection has been made on Pairly.com.

If you would like to find out what your care needs are, you can try our care calculator which helps to determine the amount and types of assistance you or a loved one may need. Our care calculator has 11 multi-choice questions, all designed around UK care assessment standards.

How do I request a care assessment?

When you have identified your chosen care provider and confirmed they have availability, they will arrange for a care assessment. It can sometimes take 1-4 weeks for your care assessment to take place, however, this time scale can differ depending on the provider you have chosen, location and needs.

What does a care assessment involve?

Health and well-being assessment

A health assessment is performed to assess a person's ability to perform daily activities independently, such as personal hygiene, meal preparation, and mobility. This assessment will also include an overview of the individual's current medical condition, including any chronic illnesses, disabilities, or specific health concerns.

They will also assess the individual's emotional and psychological well-being, including the presence of mental health conditions or cognitive impairments. There will be questions about the individual's social connections, relationships, and overall emotional state to identify potential areas of support.

Environmental Assessment

Evaluating the living conditions and surroundings of an individual to ensure they are safe, supportive, and conducive to their well-being. The aim is to understand the impact of the physical environment on the person's ability to carry out daily activities and maintain a good quality of life and ensure it is safe for care professionals to carry out care within the person's own home.

Care assessments play a crucial part in ensuring people receive the personalised care they need. Everyone is entitled to timely, quality and competent care that caters to their individual needs and enhances their quality of life which allows them to remain as independent as possible.
